Enroll / Re-Enroll Here - PowerSchool

Requirements for Enrollment

• Enroll your student via the "Enroll / Re-Enroll" button above with PowerSchool
• Original Birth Certificate (with raised seal) issued by the State – due within thirty days of registration (office personnel will make a copy for student’s file.)
• Baptismal Certificate (office personnel will make a copy for student’s file.)
• Immunization Records – must be presented before child can enter school.
• Physical Form – required for students entering kindergarten and seventh grade and students from out of state
• Court-certified copy of the custody section of the appropriate decree (if applicable)
• Create a FACTS account for tuition
• If you are registered at one of our partnering parishes (St. Mary's, Sacred Heart, St. Francis, Cristo Rey, Immaculate Heart of Mary, St. Andrew Dung Lac), you will need to get this linked Parish Verification Form signed and sealed by your parish pastor, and returned to the our school office. 
• Set up a Payment plan within your FACTS account for tuition collection
