To Do List: Requirements for Enrollment

• Preschool Application Form
• Register/enroll online and create FACTS account
• Birth Certificate – need original and then we will make copy of it and return original to you.
• Baptismal Certificate – need original and then we will make a copy of it and return original to you.
• Immunization Records
• Custodial Papers – if applicable

Note: To reserve a place for your child in the upcoming preschool class, the Preschool Application Form and the $50.00/$100.00 Registration Fee must be turned in to the school office.
